BAR HARBOR — Mount Desert Island Hospital and Jesup Memorial Library are offering help enrolling in healthcare insurance under the Affordable Care Act (ACA) at the library, 34 Mount Desert St., on Wednesday, Dec. 3, from 10 a.m. to 12 noon.
ACA Navigators will be on hand to help you enroll. Please bring the following with you:
Home and mailing addresses for everyone applying for coverage, as well as their Social Security numbers;
document information for legal immigrants (visit healthcare.gov/help/immigration-document-types for more information);
employer and income information for every member of your household (for example, from pay stubs or W-2 forms);
your best estimate of what your household income will be in 2015;
policy numbers for any current health insurance plans covering members of your household;
a completed “Employer Coverage Tool” for every job-based plan you or someone in your household is eligible for. (You’ll need to fill out this form even for coverage you’re eligible for but don’t enroll in. Visit healthcare.gov/downloads/employer-coverage-tool.pdf to view or print the tool.);
and notices from your current plan that include your plan ID if you have or had health coverage in 2014.
You have until Feb. 15 to enroll in a plan to avoid owing the per-month fee for not having health insurance in 2015. Open enrollment in the health insurance Marketplace started Nov. 15 and ends Feb. 15.
