Gap in services explored

BAR HARBOR — A “Gaps in Services Summit” was held at Birch Bay Retirement Village in February. The summit was coordinated by Debra Chalmers, executive director at Birch Bay Retirement Village, and Shoshona Smith, Healthy Acadia’s systems and innovations coordinator.

Birch Bay Retirement Village and Healthy Acadia teamed up to explore resources currently available for residents of Mount Desert Island and outer islands, specifically in the areas of chronic disease management, active older adults and palliative/hospice care services. A group of approximately 20 area health and wellness service providers joined together to become better care partners. Through a variety of real-life case scenarios, members of the group determined resources needed to support individuals with specific chronic conditions. They also identified the resources needed to support active older adults, as well as people who might benefit from palliative and hospice care. The goal of the summit was to better understand the parameters of existing resources, such as eligibility requirements, duration of services, costs associated and potential barriers to accessing services, along with common topics such as transportation and housing. The outcomes of this summit will help develop an effective resource guide to assist individuals seeking services, as well as provide a comprehensive understanding of existing services to support better care management.

This work will continue through monthly Healthy Aging MDI meetings held at Birch Bay Retirement Village in Hulls Cove on the last Monday of each month at 1 p.m. Healthy Aging MDI is an off-shoot of the Healthy Aging Task Force through Healthy Acadia, which meets in Ellsworth every other month on Mondays at noon in the Moore Community Center.
